Unable to verify bank account - Amazon is asking that my personal name be included on LLC bank statement

I am unable to verify the bank account as mandated by the INFORM Act requirements. The bank account is held by the LLC, but for some reason Amazon is requiring that my personal name be on the bank statement. Amazon's systems show that the account owner is the LLC, as well as the tax information (is the LLC). However, when I try to verify the bank account information, the only option for Bank Account Holder is my personal name.
I have reached out to Amazon Seller service - but nobody seems willing (or able?) to resolve this issue. Case ID#13010584851 and second attempt Case ID#13032976171

It appears (from other posts) that Amazon has been aware of this problem for over a month and has not been able to resolve it. It seems rather unfair that they threaten me with deactivation of an account that has been in good standing for 10 years due to a systems issue on their side.

I can only select my Point of Contact name on the Bank Account Verification page. What do I do?
We’re aware some sellers are only able to select their Point of Contact name on the Bank Account Verification page. If you don’t see any other options, you can select the Point of Contact name and submit your bank statement as is. If the Bank Account Holder name on your bank statement matches the ‘Legal Entity Name’ listed under Tax Information, we can use this to complete your verification.
