Refunds - Why are they being Automatically given?

Since when has Amazon been given refunds automatically? I have always authorized a refund, once received the item then issued a refund, i am discovering Amazon are already issuing them before i have received the goods back?

You have obviously missed the email / new procedure where items are refunded on first scan.
