Seller verification issue

I am here to ask for some guidance. I am in the process of having my business verified and now I need to provide proof of address for Primary Contact.
I uploaded bank statement --> got verified and then two days later suspended again. Reason - unable to verify the document I provided.
Well now I am back to square one and I am asked to upload Proof Of Address. However, under the comment section, it says:

Proof of Address - Bank account statement
- The identity document is not acceptable.
Proof of identity - Passport
- The middle name on the identity document does not match the middle name entered in Seller Central, or is missing.

I have been seller since Spring last year and been hit with verification after passing 15k limit. I opened LTD company, and having issues getting my Payments Account verified.
I guess my question is - should I open a case with Seller Support asking for clarification, or, perhaps any of You could help me out here.
As I understand, I need to upload Proof Of Address, but why would it say that my passport information does not match the details I typed in when creating the account, even though I was selling for over half a year.

Your details need to match EXACTLY. If your name is David Jones in your seller account and David B Jones on your passport = then it doesn’t match.
Usually the bank statement -( so long as it shows your full address and it matches EXACTLY to your details on Amazon) is sufficient. Does it match?

In the e-mail, it states:
‘‘The bank statement you provided was not 1 of the acceptable document types.’’
As I understand, I need to re-upload any 1 of the following:
– A copy of a proof of address document showing the individual’s name and residential address. We can accept a council tax bill, if valid for the current year, or any of the documents below, if dated within the last 180 days:
- Utility bill (gas, water, electricity, TV, Internet, mobile phone, or landline)
- Bank statement
- Credit union or building society statement
- Credit card statement or bill
- Mortgage statement
- Rent receipt from a local council or letting agent
- Benefits agency letter (for example from the Department for Works & Pensions, Job Centre Plus, or Veterans Agency) confirming your rights to benefits.
Am I understanding this correctly?

Sorry if this seems confusing, but please bear with me.
My account got re-instated, and one day later suspended again, stating that ‘‘payments account has failed verification process’’.
Please note that his happened twice, that I got verified, and shortly after suspended again.
What gives? I sent email to seller performance team but have not received any response for a week now. Also, my case status shows as ‘‘transferred’’…
Furthermore, I tried appealing and interestingly, I am allowed to add additional documents to the appeal, however, Amazon never asked for any more documents.
What can I do at this point?
