Does every individual item need a manufacturer bar-code or Amazon bar-code?

When Amazon mentioned items must have a manufacture bar-code or an amazon labelled bar-code, does that mean physically on every individual item. And if you go down the manufacture bar-code route, is that the responsibility of the manufacturer or Amazon to place on the individual goods ?
