How do I reactivate my account? What is such document?

Help!
I am opening an account in Amazon. But it is deactivated, and received the message as below.
I tried several times of submission, but all failed.

1. I am opening a business account.
   What is the "your name“ in below statement refer to? “The company name” or my name “the owner”
   of the company?

2. If any idea about the document which are being asked for , please help to advise.
