Reporting a copyright infringment issue

Hi everyone,

I'm trying to report a copyright infringement using this form for non-brand registered sellers:

I filled out everything they asked for — my name, phone number, address, a link to my design, the verification code, etc.

But I keep getting the same email reply from Amazon:

“We reviewed the additional information regarding your identity and could not verify the information you have provided. If you believe there was a mistake, please resubmit the notice with any additional information requested and the verification code below.”

I’ve included the verification code, but I honestly have no idea what else they need to verify my identity. Has anyone else dealt with this? Any tips?
