Read onlyI have never held multiple seller accounts. The other account that Amazon suspects to be mine was opened by my brother. I used his physical home address for buyer returns due to security at his house. I also accessed my seller central account from his computer at his residence using his IP address. I have submitted a sworn, witnessed and signed by a lawyer affidavit, and documents showing the other account was registered by my brother who has a different first name. However, Amazon keeps declining my deactivation request. Could the Amazon team here please help with this matter? Thanks.

As noted this would be a reason that the association was established with your brothers account. The notification you received advises on the types of documentation we can accept that shows why there was an association. These types of document establish a business association with you and help us separate the association, however when you have such a close non business related connection these documents may not exist.
You mentioned providing an affidavit along with his registration information, regarding the affidavit, this will only be accepted in situations where partial supporting documents exist.
Example: You have contracts that you hired someone but cannot show proof of termination. An affidavit can be used to verify proof of termination, be aware we may request additional documentation such as bank statements as confirmation of payment and when it stopped as proof.
As this does not appear to be the situation, an affidavit would not be a valid path in this situation.
Registration information only shows he owns his account, not that you're not associated with them. As described, you used his address and device establishing a non business related connection which can only be resolved by ensuring his account is reactivated as mentioned by @Seller_OvL8C4BJWiuS9.
