I mean it seems every shipment there is an issue now. Anyone else notice? Wonder if Amazon HQ have?
Do the warehouse operatives read the "send to amazon" workflow?

No more than usual for us. Just the standard incorrect unit quantity - thankfully we sell low value items so we have always just accepted it.

I tried FBA, but gave up on it.
My situation is likely different to yours, but FBA was more hassle than it was worth.
Lost shipments, miscounting, and targets to hit etc.

Last few weeks they have temp workers for the summer holidays lots of people on leave. A very busy period as end of Season loading for alot. I noticed sold out high ranking items taking 2 weeks to check in, instead of 1-2 days. Maybe this busy period has affected the inbound issues?
If you check the tracking you can see when a delivery was attempted.
Amazon refuse UPS Deliveries to save space they put them in holding then UPS bill them for storage. Maybe they have done that with yours causing issues?
