How to appeal a Customer reviews policy Violation

I need your help how to write prompt Plan of Action for How to appeal a Customer Product Reviews Policy Violation. I appealed 2-times but amazon rejected my request. please help me my account is at risk.

You might need to post the reason for the violation and the current plan of action to get suggested improvements. Though at the end of the day, no one has a silver bullet that can allow the appeal to be approved.

I believe they like a POA in a 3 point format, at least they used to .
Am sure if you search the forum there will be some guidance. I did try searching Amazon, but there is nothing I can see in their guidelines.
If I recall, the 3 points are Details of Root Cause, Actions Taken, and Preventative Measures so doesn't happen again - or something along those lines (!).
