Royal Mail Surcharge Appeal Link

I know this must have been asked before and I have used it myself, but does anyone have a link to the Royal Mail surcharge appeal form? I have tried searching Google and the Royal Mail suite and can't find anything.

Note, I have the lost/damaged claim form but the surcharge form is a different one.
