Two features of interest here.

The first one is remove your personal information (blank it out)

The second one is, what are you providing an LOA for?

Yes, I was in rush and didn’t blank full address…

Reason for LOA:

Home page alent:

Alert

Flip Card

Additional information required

Additional information is required to open your Selling on Amazon payment account.

Then it is asking Letter of authorisation issued to Joshua Justin Pereira (me) from the company which I have submitted and I believe it follows as per : Amazon

Did the right person sign it? It should have been Clare Haynes who signed it.
Also is the director named within this consistent with the beneficial owners info that you will already have given Amazon?
