EC Declaration of Conformity
You must submit all required documents. Your products cannot be approved if some documents are missing. EC Declaration of Conformity and Genuine Product images have to be submitted as separate file types.
Document must meet the following requirements:
Name and full business address or that of your authorised representative ( what would you put there ?? the wholesaler or our details ??)
Product’s serial number, model or type identification ( what you put here)
Should state that you take full responsibility for the declaration. Should show the relevant legislation with which the product complies, as well as any harmonised standards or other means used to prove compliance. (what would you do for this )
Name, signature and position of person signing ( is this for use for us to put our name and signuter??
The date on which the declaration was issued

Usually an EU-based manufacturer or importer will create a DoC for each product they supply, so it might be best to contact them first to see if they have one available. A lot of Chinese manufacturers also create them in advance for convenience.
That being said if you still need to create your own:

The model number of the product, the ASIN/SKU/EAN is also acceptable, also include a description of the product (match the Amazon listing title as closely as possible)

Now this is the hard part:
You will need to do some research to confirm which legislation your product complies with, from my understanding the harmonised standards are optional but show that your product goes above and beyond. You usually only need to list the identifier for the specific parts you comply with, for example:
2016/426/EU
(this is the regulation that you must comply with if you sell a heavy machine that burns gaseous fuel)
Although some people do put notes after each line for ease of reading, for example:
2016/426/EU - regulation for gas powered machinery.
There’s no specific format that is required. Many companies even leave out the list of standards but this would not be accepted by Amazon.

Just remember that the legislation and standards are different for each specific product, many of them are based around the use of certain materials,
