How to list a product on amazon

Hello, I have 4 products these products are liquid, i put these products in a polybag. I have listed this product but am stuck on the Contribution SKU what does this mean? does this mean 4 products so i have to input 4 in the box? Which box should i choose, list as fba or list as fba and send to amazon?
