Unable to Reactivate Account with Utility Bill - Multiple Attempts
I get the following Prompt to reactivate my account:
To reactivate your account, please confirm that there is a valid credit card on file and provide one of the following documents in PDF, PNG, JPEG, or GIF format. The documents must be legible, authentic, and unaltered and contain the required information. We do not accept screenshots. The name and address on the document must match the information entered in Seller Central.
-- A business license if applicable
-- A utility bill dated within the last 90 days for piped gas, natural gas, electricity, piped water, mobile phone, fixed-line phone, or internet service with name and address visible
I keep submitting my phone bill from 3 which has my full name and address on it.
I get one of the following responses:
1. We were unable to complete the review because you did not provide the documents required for the appeal.
or
2. We were unable to verify the documents that you provided.
I have checked and the document seems to meet the requirements of Amazon. No one will tell me why it is not accepted nor what they expect me to upload. This has been ongoing for 2 years now and I have uploaded documents many times, which Amazon never accepts.
Can someone please help, I have emailed amazon many times and no one will respond to my case.

I have submitted utility bills many times and they are never accepted. They meet all the requirements listed on Amazon's notes (full name, address, etc). I have also contacted Amazon many times and no one responds.
I am never given a reason nor a specific answer, just the above automated responses.
My husband's account has now been deactivated as Amazon claims it is related to this one (other than us being married, there is no business relationship between the two accounts. I do not have access to his and he does not have access to mine).
He has submitted a signed and notarized affidavit stating this (as amazon requested) but they have not accepted it. Claiming that I need to fix this issue first. Neither of us have anything strange with our identities, all very standard stuff (passport uploaded, utility bill and bank account linked).
Can someone from Amazon please help??

Was your bill a PDF download?
Amazon don't like those (only amazon are allowed to be paperless guys!)
If it is, print the PDF out, fold it a few times, then scan it back in.
Can't guarantee that will fix things, but I know its been a known fix in the past.

3. Make sure the above files are in .pdf, .png, or .gif format.
If any of the bills are available in your family member’s name, kindly provide a relationship proof like a govt. issued ID with address.
These documents must be authentic and unaltered, and the name and address on the document must match the name and address you provided during registration.
There must be some information that was missing or not similar to seller central details or the document was not valid.
