Phantom A-Z Claims that keep reappearing as granted after they have been withdrawn

We are having an issue with two A-Z claims that were both granted and then both subsequently withdrawn by the respective customers. In both cases each claim reappeared as granted after 3 days and without any kind of notice. We have confirmed with both customers that it was not their doing and this is unfortunately not the first time this has happened to us. In regards to order ID: 113-5550041-5057817 the customer withdrew the claim on 3 separate occasions, then each time, 3 days later it would automatically reappear as granted, the case number for this order is (case ID 15309046981). In the most recent instance with Order ID: 111-0776704-2207423 the exact same thing happened and the claim was again regranted 3 days after it was withdrawn by the customer. We again opened a case (case ID 15436580121) for which we received no response. For both these orders we have lost the product, the funds and most importantly both orders are incorrectly and negatively affecting our Order Defect Rate or ODR. If someone could please escalate these orders/cases and this ongoing issue to the A-Z claims team for review, we would greatly appreciate it. Thank you-

Chiming in to let you know I connected these orders to our partner team to request some further review and assistance. I will keep you posted with any developments I am made aware of, but in my experience these reviews can take some time.
To help with future such scenarios, I will share this guidance on how to prevent A-to-z Claims, along with what to include when appealing claims.
