All self-delivered items have been fully refunded
I listed a product in March, but on April 5th all orders were fully refunded to the buyer, but the buyer did not apply for a refund. Has anyone encountered this situation? How can I solve it.

What do you mean by 'self delivered"? Do you mean that you drove to the customer's house and dropped it off? Or that you did FBM?
If you did FBM, did you buy shipping through Amazon? Or through another source? If another source, did you upload the tracking info to Amazon?
If you did not buy shipping through Amazon, and did not upload legitimate tracking numbers, then after a certain amount of time, Amazon will reimburse the customer, as they have no indication that the item was shipped.

I have looked into a few recent orders and noticed they were refunded automatically. For eligible returns, orders equal or below $25 Amazon will automatically provide a returnless refund, see our help page on Customer returns for international sales:
Automated returnless refund: If you are fulfilling customer orders in the US from outside the US and a default US return address is not provided, Amazon will automatically issue the customer a returnless refund for eligible returns equal to or under 25 USD without requiring the item to be returned.
